Case Manager - Hohenwald, TN

JOB TITLE:  Case Manager, Full Time                                   

LOCATION: Hohenwald TN (West Facility)

SCHEDULE:  1st Shift                                                             

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.  Other duties may be assigned.

  • Orients new clients as to goals and objectives of program, role of the case manager, and services that are available and accessible to the client
  • Completes Initial Admission Assessments and Admission packet on all clients assigned to them within 36 hours of entry
  • Assists clients in obtaining employment and permanent housing
  • Facilitates weekly resident council meetings in transitional housing
  • Monitors transitional houses for cleanliness and compliance to safety rules
  • Maintains up-to-date, accurate and understandable case files and records, as delineated on the Chart Order Checklist
  • Develops employment and housing referral network and maintains professional relationship with employers/landlords to ensure continued coalition
  • Coordinates transportation of clients to ensure client reaches destination by scheduled time frame
  • Completes all necessary Voc. Rehab. requirements/documentation, related to contractual agreement.  May include performing Job Readiness training
  • Attends scheduled team meetings and training sessions
  • Conducts follow-up on all assigned clients for a minimum of one (1) year
  • Prepares monthly/quarterly reports of activities and submits to supervisor on scheduled due date

QUALIFICATIONS

  • Strong computer skills.  Ability to type 30 wpm.  Proficient with Microsoft Office Programs and Electronic Health Record software
  • Able to communicate both verbally and in writing
  • Pass Background Check and Drug Screen required
  • Keeps a current CPR and First Aid certifications, required
  • Valid TN D.L. with "F" endorsement required; can be obtained once hired

EDUCATION and/or EXPERIENCE

  • One-year certificate from college or technical school; or 6 to 12 months related experience and/or training; or equivalent combination of education and experience as well as previous experience working in chemical dependency field preferred
  • High School Diploma or GED
